How did the RS mod know it wasn't a rehash of the recipient's prompt/letter if the gift was too squicky for them to read in the first place?

Because they asked for a couple of volunteers on the mod server to read it for them and help them make a judgment call, which is what OP of this entire thread is complaining about.

My question is if they rejected it by themselves the first time and then asked the server for the second rejection, or if they asked the server for both rejections.

They asked the server both times, even though they'd already decided no EPH. That's why it felt so cruel: the mod had already made up their mind, yet they were inviting additional scrutiny that wasn't constructive.
