At this point we know for sure it was a bug. AO3 is treating it as a bug (and from what I've heard even investigating reports about this from other exchanges from earlier in the year that they weren't responding to until now) and there was at least one completely unrelated mod of a smaller exchange on EAD who had the same thing happen and was able to post screenshots with everything clearly checked right.
