In a written medium, it doesn’t work to try to describe every visual detail every time, either. If a character in a tv show has a frequent nervous tic, you mention it once or twice in the fic, where it makes the most impact. If you mention it every single time it would have happened on screen if the fic were an episode of the tv show, that’s tedious and annoying. The same is true of italicized speech pattnerns.

Then you find a way to describe it in varying ways. That's how writing works. If the nervous tic is that omnipresent on TV, it's meant to be noticeable and disruptive and you have to get that across in your fic as well. If you write them do it once or twice with no indication that they constantly do it, then it's OOC.

As often as it happens. Which is probably not half as often as you think. If the character rubs his neck when he's nervous, he's not going to do that every time he shows up or says a word, because otherwise his neck would be bleeding halfway through a movie.

How would you deal with TOS fic with Kirk? Shatner has a very distinctive speaking style but I find fic that essentially transcribes him unreadable.

I have no idea, sorry. I've watched TOS many times when I was young, but always dubbed, so I was deprived of the dubious pleasure of hearing Shatner's original voice, and I never read any fic for it, so I don't know how it's usually handled.

I used to be in a fandom where half of the juggernaut had a very distinctive (and very dramatic) speaking style that involved a lot of emphasis, and people always liberally used (and still do, when I go on a nostalgic reading spree) italics for his dialogue, which seems to work for most people in the fandom.
