The thing is that omegaverse is built off a lot of the tropes of sexist oppression, which is something that it's likely that your recip has had some experience with. You have a greater chance of hitting a recip who's been sexually harassed or sexually assaulted or hit a significant barrier due to sexism than to hit a recip with eating disorders.
I don't think omegaverse is "so endemic to fic culture" at all. It's a large AU in some fandoms. That's it.
Omegaverse is still a fantasy trope though where the sexual oppression that exists is several layers removed from the real world. Eating disorders are very much real world issues.
Also, omegaverse is fairly popular, eating disorders popping up in fic when there's no canon basis for it is vanishingly rare. So omegaverse is much more in the "they should know it's not unlikely to appear and DNW it if they don't want it" than random eating disorder fic.
Eating disorders therefore presumably exist in the world of the canon, while omegaverse does not. It's therefore far more of a step away from the canon setting and situation. It's precisely this lack of canon basis that means that people shouldn't have to specify it as a DNW as they shouldn't be at risk of getting it unprompted. Just like they shouldn't be at risk of getting the fic where A and B and C are part of a swarm of bees and D is an anteater.
I just disagree with this approach to both fic content and DNWs. I DNW AUs because they're a big part of fanfic culture and if I didn't DNW them, I think the author wouldn't be wrong to write them.
I agree about AUs in general, but not about all AUs. Canon divergence and setting AUs are a big part of fanfic culture. AUs which create a whole alternate system of reproduction and discrimination are not.
If you're in Supernatural RPF fandom, which originated omegaverse, maybe it's common enough to be a huge risk. In most fandoms, things like omegaverse and hanahaki or whatever crazy biologically deterministic AU is rising in some other fandoms, are not the same.

Citation needed, in most omegaverse fics I've read you can find/replace "omega" with "woman" and it just reads like someone's slightly iddy AU of real misogyny.
There's tons of omegaverse where I'd argue this isn't the case, I personally know a lot of trans people who are writing their own feelings into omegaverse in a dozen different ways, from filthy porn to gender issues.
But what pretty much all of it has in common is that some degree of real-world gender, sex, and gendered-and-sexual-violence comes into play, plus there are dog dicks, so people's desire to avoid it/its status as an AU not everyone is willing to read should be blisteringly obvious.
As someone who likes omegaverse I would feel weird gifting it to anyone unprompted, especially for yuletide, because of how much omegaverse usually revolves around the idea that the featured characters are gonna bang in some way, never mind the question of how much it leans into the omegas=women thing.
