Is 'on accident' a purely American error? Because it drives me, an American, nuts.

It's a regionalism within America. Some parts of America use "on accident" instead of "by accident" almost exclusively, some use both interchangeably, some only use "by accident." So yeah, it's not surprising that some Americans would hate it more than others.

Southern U.S. coalie here-- "on accident" sounds wrong and vaguely silly to me, sort of like when people say "how come" instead of "why." "By accident" sounds correct.

Yes, the phrasing you use habitually will usually sound correct to you. Doesn't mean other regional dialects are wrong or silly.
