I really want to comment on this fic to compliment it on its prose. It really was very well-written!

Except the very first sentence has a typo in it. Should I say something? Or just comment positively without mentioning the prose?

One typo does not worse prose make. You should still comment positively about the prose! But don't mention the typo. They'll probably see it and fix it eventually.

This is really strange framing. Having a single typo in the first sentence does not negate good prose. Something can be well-written but riddled with typos; something can be perfectly grammatically correct and still have crappy prose. Yes, typos are annoying, but I would be aghast if a single typo genuinely ruined your enjoyment of the overall prose. Everyone is different with regards to whether or not they like having typos pointed out, but you can still compliment the prose either way.

I personally think spelling & grammar is necessary but not sufficient for good prose.

If you had a cheap edition of a classic novel with typos in it, would you say that it had worse prose than a different copy?

Actually, yes, from a purely reading standpoint. A deep analysis, no, but as a casual reader, the SPAG is going to take away from the strength of the prose as I read it.

I personally would want someone to let me know. Perhaps write your comment with an extra sentence pointing it out, then go back and immediately edit the comment, deleting that sentence?

They’ll get the original comment in an email, but no one else will see it.

Point out the typo. There is no excuse for a published fic (yes, Ao3 is publishing) with a typo in the first sentence.

oh god, please tell me about any typos I make in a giftfic! I'll be embarrassed, but also relieved it was quickly caught so I could fix it.
